About The Position

Overview To enhance sepsis outcomes and sustain compliance with evidence-based protocols, this proposal recommends the creation of two part-time Sepsis Navigator positions – one focused on data analytics and one dedicated to clinical engagement. Background Sepsis continues to be a leading cause of morbidity, mortality, and hospital readmissions. Early identification, timely intervention, and consistent process monitoring are critical to improving patient outcomes and meeting bundle compliance. Current resources are limited in their ability to simultaneously manage real-time clinical oversight and data-driven performance improvement.

Requirements

  • RN-Bachelor’s Degree or BLD- Bachelor's Level Degree or RN-Master’s Degree
  • Registered Nurse (RN) Single State - Nursing License - Virginia Department of Health Professionals (VADHP)
  • Registered Nurse (RN) Multi State - Nursing License - Virginia Department of Health Professionals (VADHP)
  • Physical Therapist License (PT) - State license - Virginia Department of Health Professionals (VADHP)
  • Physical Therapist Assistant License (PTA) - State license - Virginia Department of Health Professionals (VADHP)
  • Licensed Nurse Practitioner (NP) - Certification - Other/National
  • Physician Assistant License (PA) - State License - Virginia Department of Health Professionals (VADHP)
  • Physician Assistant License (PA) - State License - Other/National
  • Certified Occupational Therapy Assistant (COTA) - Certifications - National Board Certification In Occupational Therapy (NBCOT)
  • Occupational Therapist License (OT) - State license - Virginia Department of Health Professionals (VADHP)
  • BS Degree required or in lieu of degree: 5-10 years of relevant work experience and candidate must obtain BS degree within 18 months of hire. For RN candidates, BSN required.
  • 3 years clinical experience
  • Registered nurse, Physical Therapy, Physical Therapy Assistant, Occupational Therapy, Occupational Therapy Assistant, Nurse Practitioner or Physician Assistant license required.

Nice To Haves

  • Preferred in patient care setting of orthopedics, neurosciences or musculoskeletal.

Responsibilities

  • Sepsis Navigator – Data Analytics Focuses on data abstraction, trend analysis, chart reviews, case follow-up and reporting
  • Identifies performance gaps and supports continuous improvement initiatives
  • Works collaboratively with IT, quality, Clinical Sepsis Navigator, and the clinical teams to track compliance with SEP-1 measures and hospital metrics.
  • Sepsis Navigator – Clinical Provides direct support to bedside teams for early sepsis recognition and bundle implementation
  • Conducts real-time chart reviews and case follow-up
  • Engages with frontline staff to reinforce best practices, offer education, and ensures timely interventions
