About The Position

The Senior Integrated Behavioral Health Consultant provides behavioral health assessment, consultation, treatment, care coordination, and clinical leadership within an integrated healthcare setting serving patients across the lifespan. This role functions as both a Behavioral Health Consultant and a Senior Integrated Clinician. The clinician provides consultation to medical providers, performs intake assessment, delivers brief interventions, coordinates care, and facilitates referrals to appropriate services. In the Senior Integrated Clinician function, the primary clinical modality is group therapy. The clinician develops, implements, and facilitates evidence-based behavioral health groups designed to meet the needs of the population served. This position also provides clinical leadership, program support, and promotes the integrity of the integrated care model.

Requirements

  • Master’s degree in Marriage and Family Therapy, Counseling, Social Work, Psychology, or related license-eligible field.
  • Minimum of two (2) years post-graduate clinical experience required.
  • Licensed in the State of Connecticut (LCSW, LMFT, LPC or Licensed Psychologist)

Responsibilities

  • Works as part of a multidisciplinary team to efficiently and effectively meet the behavioral, primary, and dental health needs of the client, including providing behavioral health screening, assessment, consultation, treatment, and care planning to patients seen by medical providers.
  • Provides brief behavioral health interventions using evidence-based practices to support improved health outcomes and functioning.
  • Assists the medical team in developing care management processes including patient education, behavioral interventions, and disease management strategies to improve patient self-management and treatment adherence.
  • Conducts screening, evaluation, and formulates diagnoses in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents clear, individualized, and measurable treatment plans.
  • In the Behavioral Health Outpatient setting, provides group therapy as the primary modality of treatment, including development, implementation, and facilitation of evidence-based behavioral health groups that address the clinical needs of the population served.
  • Utilizes evidence-based group interventions to address mental health, substance use, and behavioral health conditions.
  • Demonstrates knowledge of psychopathology and behavioral health treatment modalities and appropriately triages clinical emergencies.
  • Demonstrates knowledge of internal and external levels of care and makes clinically appropriate referrals.
  • Provides warm handoffs and coordinates care with internal and external providers.
  • Communicates in a positive, timely, and effective manner with clients and treatment team members.
  • Meets direct service productivity goals as defined by program.
  • Documents all client-related information in compliance with agency policy.
  • Attends and is prepared for supervision and required meetings.
  • Provides clinical leadership by promoting and sustaining evidence-based and best practice models of care.
  • Provides consultation, guidance, and support to staff as assigned.
  • Collaborate with leadership to identify program needs and opportunities for improvement.
  • Participates in program development, quality improvement, and performance monitoring activities.
  • Serves as role model for staff to promote strong work ethic and adherence to integrated care practices.
  • Provides culturally competent care and maintains sensitivity to and respect for clients’ cultural traditions, values, and beliefs.
  • Interfaces and collaborates with internal and external providers to support coordinated patient care.
  • Reports abuse and neglect as mandated and adheres to confidentiality guidelines.
  • Completion of all required training related to Integrated Health Care as assigned.
  • Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
  • Understands and adheres to Wheeler Clinic’s compliance standards as they appear in the clinic’s Corporate Compliance Plan, Code of Conduct, and Conflict of Interest Policy.
  • Completes all training in a timely manner as required by state and federal regulations (e.g., Compliance, Fire Safety, HIPAA HITECH, Infection Control, Sexual Harassment, etc) and specific to the department and position.
  • Keeps abreast of all pertinent federal, state and clinic regulations, laws, and policies as they presently exist and as they change or are modified.
  • Ensures that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs.
  • Performs other related duties as assigned.
