About The Position

We are seeking an experienced and compassionate Senior Healthcare Liaison to join our dynamic team. This role is pivotal in ensuring effective communication and coordination between healthcare providers, patients, and their families. As a Senior Healthcare Liaison, you will be responsible for facilitating seamless transitions of care, advocating for patient needs, and enhancing the overall patient experience within our organization.

Requirements

• Bachelor’s degree in healthcare administration, nursing, or a related field; advanced degree preferred.
• A minimum of 5 years of experience in a healthcare coordination or liaison role.
• Strong understanding of healthcare systems, patient care practices, and regulatory requirements.
• Excellent communication and interpersonal skills, with the ability to handle sensitive situations with empathy and professionalism.
• Proven ability to work collaboratively in a multidisciplinary team environment.
• Strong organizational skills and attention to detail.
• Proficient in using healthcare information systems and technology.

Responsibilities

• Serve as the primary point of contact for patients and their families, addressing concerns and providing guidance throughout their healthcare journey.
• Collaborate with healthcare providers to coordinate patient care plans, ensuring that all parties are informed and aligned on treatment goals and progress.
• Conduct regular follow-ups with patients post-discharge to ensure continuity of care and address any ongoing health concerns or questions.
• Develop and maintain strong relationships with healthcare partners, including hospitals, clinics, and other service providers, to enhance the quality of care delivered.
• Identify opportunities for process improvements and work collaboratively with internal teams to implement changes that enhance patient satisfaction and care outcomes.
• Educate patients and their families on available healthcare services, resources, and support systems to empower them in managing their healthcare effectively.
• Ensure compliance with all relevant healthcare regulations and organizational policies.
