The Senior Enrollment Specialist is a member of the Clinically Integrated Network's Ambulatory Care Management team and is accountable for patient enrollment and support of patients in the program. The Senior Enrollment Specialist works as part of a multi-disciplinary Chronic Care Management team under the direction of the Ambulatory Care Management leadership. The Senior Enrollment Specialist will serve as the point person for the complex triaging of new patient referrals and downstream communication processes. The Senior Enrollment Specialist is responsible for performing patient outreach to educate and enroll patients into the program. Once enrolled, the Senior Enrollment Specialist supports the Chronic Care Managers on patient tasks to meet the needs of the patient.

Requirements

• High school diploma or equivalent required
• Two to four years of customer service experience in a patient care area of a healthcare environment
• At minimum one year of successful medical call center experience required
• Experience and familiarity related to symptom assessments with documentation in the electronic medical record based on call presentation preferred
• Excellent oral and written communication skills
• Basic typing and keyboarding skills
• Possesses excellent organizational skills and ability to handle multiple priorities
• Ability to work in an independent role with minimal supervision
• Functions as an integral team member and demonstrates flexibility in sharing responsibilities
• Working knowledge of computers and basic software applications used in job functions, such as word processing, databases, spreadsheets, and others as needed

Nice To Haves

• Associate or bachelor degree preferred

Responsibilities

• Completes enrollment outreach calls to eligible patients
• Manages tasks delegated by the interdisciplinary team, including but not limited to transportation needs, appointment needs, referral needs, and other patient navigation items
• Identifies and refers patients to RN Triage for assessment if needed
• Escalates any issues requiring clinical decision making or clinical assessment to the RN care manager or leadership as applicable
• Demonstrates an ability to serve as a collaborative member of a multidisciplinary healthcare team
• Assists the care management team to streamline the delivery of service
• Contacts and coordinates with referral agencies to arrange the provision of ordered equipment and associated services when appropriate
• Able to relate and communicate positively, effectively, and professionally with others
• Responsible for running and maintaining the eligibility list patient registry reports
• Engages patients and/or caregivers regarding the opportunity to enroll in this program
• Reviews & educates patients in the use of MyChart
• Documents telephonic interaction in the medical record per the standard of the department
• Serves as the point person for complex triaging of new patient referrals into the program and the associated communication with the requestor and patient
