About The Position

Provides direct service to assigned caseload of individuals that have been arraigned and have been released into the community as their court case proceeds through the legal system. Services include comprehensive assessment, service planning, coordination of stabilization needs and other identified areas of need or interest, recordkeeping, case management, collaboration and outreach.

Requirements

  • Demonstrated ability to work independently and to initiate change
  • Must possess excellent skill and judgment in a range of clinical and case management activities
  • Must be able to collaborate effectively with a team, DOCCS area office staff and have superior interpersonal skills in dealing with other providers and client supports
  • Must possess excellent written and verbal skills
  • Proficient in the use of computer software such as MSWord, Excel, Power Point, Desktop Publishing, Outlook, etc.
  • Must possess excellent time management skills.
  • Must be able to meet deadlines with documentation.
  • Associate or bachelor’s degree
  • At least one year of Case Management experience with a bachelor’s degree OR
  • Two years’ experience with an associate degree
  • At least one year of supervisory experience.
  • Must possess a valid Driver’s License with a satisfactory driving record, and possess a personal vehicle for job requirement

Nice To Haves

  • Preferred two years experience working with individuals involved in the criminal justice system.

Responsibilities

  • Knowledge and demonstration of agency core values in day-to-day activities
  • Provides comprehensive case management assessment to assigned caseload
  • Work effectively with probation and other community partners.
  • Work effectively with individuals enrolled to meet benchmarksregardingenrollment,linkage,disenrollmentand90 day follow up.
  • Assure all individualized Action Plan (IAP)includes linkages to needed services to be achieved in the anticipated 90 days enrolled with this program.
  • Provide outreach to client, family, and other involved persons
  • Assure and maintain linkages to medical, financial, social, rehabilitative, addiction and psychiatric services
  • Provide effective advocacy both to obtain services required by individual clients and to obtain needed systems development or change this may include meeting individuals at their appointments if needed
  • Serve as a social change agent by identifying service barriers, participating in the development of solutions, and serving as a systems advocate
  • Complete all required data entry into the OMH data system.
  • Complete all required documentation in the Erie County Probation database within the required 24 hours.
  • Provide supervision to other team members working in the program.
  • Have ongoing communication with Erie County Department of Mental Health as well as the NYS Office of Mental Health as needed.
  • Participate in all scheduled meetings with OMH and Erie County.
  • Assure data is complete and accurate for the program as a whole.
