About The Position

The Senior Community Health Nurse provides nursing care and applies nursing principles and techniques in the care and treatment of patients within various programs including Improved Pregnancy Outcome, Family Planning, Communicable disease, Immunizations, Refugee Health, Sexually Transmitted Infections/Diseases, and Chronic disease. The role involves performing independent professional public health nursing duties to foster and preserve good community health, interviewing, counseling, and teaching clients according to assessed needs and program requirements. The nurse will also perform case management duties, communicate effectively with clients and staff, utilize electronic health records, and participate in continuing education and staff meetings. The position requires working independently in clinic settings and utilizing community resources for client referrals.

Requirements

  • Knowledge of nursing principles, nursing care, and the nursing process.
  • Knowledge in assessing patient health problems and needs.
  • Knowledge in developing and implementing nursing care plans.
  • Knowledge of computer software including EHR.
  • Knowledge of Public Health Practices.
  • Ability to administer nursing care and advise patients on health maintenance.
  • Ability to perform case management and provide ongoing assessment.
  • Ability to perform physical activities including lifting and climbing.
  • Ability to lift 20 pounds.
  • Ability to read and apply applicable rules and regulations.

Nice To Haves

  • At least one year of RN experience.
  • At least one year of Public Health Nursing experience.
  • Experience working in Women’s health and Family Planning.
  • Experience working in Obstetrics/Maternity Care.
  • Experience working with immunizations.
  • Bachelor’s degree or higher in Nursing.

Responsibilities

  • Provides nursing care and applies nursing principles and techniques in the care and treatment of patients.
  • Performs independent professional public health nursing duties.
  • Interviews, counsels, and teaches clients according to assessed needs.
  • Performs professional public health nursing duties including education, assessment, counseling, and treatment.
  • Completes case management duties including follow-up care and referrals.
  • Communicates with respect and understanding with clients and staff.
  • Utilizes electronic health records accurately.
  • Participates in continuing education and staff meetings.
  • Completes required time management documentation.
  • Assists in the preparation of clinic rooms and equipment.
  • Completes clinical audits to ensure proper delivery of services.
