Requirements

  • Master's Degree from accredited school of Counseling OR Master's Degree from an accredited school of Social Work
  • 5+ years in provision of chemical dependency services, including: partial hospitalization, intensive outpatient programming, aftercare services, group and individual therapy (Required)
  • Prior outpatient chemical dependency experience (Required)
  • Prior experience in program and educational development (Required)
  • Prior Supervisory experience (Required)
  • Familiarity with community support agency resources (Required proficiency)
  • Certification as CCDC-IIIE by Ohio Department of Alcohol and Drug Addiction Services, eligibility for licensing as LICDC. (Required)
  • Licensed Social Worker (LSW or LISW) in the State of Ohio OR Licensed Professional Counselor (LPC or LPCC) in the State of Ohio

Nice To Haves

  • Interest and/or experience in working with young adults (age 18 - 25) is strongly preferred.

Responsibilities

  • Develop and present educational programs for outpatient chemical dependent programs
  • Provide direct services to outpatient chemical dependency patient.
  • Provide individual and group therapy to program participants
  • Develop and implement individualized plans of care
  • Participate in interdisciplinary treatment planning
  • Assist patients with aftercare and discharge planning for community support
  • Ongoing participation in program development
  • Participate in Quality Management activities and initiatives
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
