Don't miss out on being a part of something special! General Description The Senior Case Worker is responsible for the coordination of services within the Therapeutic Foster Care Program. This role combines leadership, compliance oversight, and direct service to ensure quality care, regulatory adherence, and positive outcomes for children and families.

Requirements

Bachelor’s degree in Social Worker or related field preferred
Must be 21 years of age
At least 3 years of experience in child welfare, foster care, mental health, or service coordination required
Accommodates daytime travel within a six-county service area, with reimbursement for mileage.
Possession of a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
Continuous use of a reliable, registered and insured vehicle.
Ability to work in the following software programs: Windows, Microsoft Office, and multiple online software platforms as required by funders and partner organizations
Ability to maintain confidentiality and handle confidential information with discretion
Ability to prioritize assignments, plan, and complete work projects with minimal direction
Exceptional time management skills and ability to effectively and meet deadlines
Excellent verbal/written skills
Ability to demonstrate behaviors and attitudes which support organizational mission, philosophy, and policies
Ability to work in a cooperative and helpful manner with all individuals
Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques

Responsibilities

Serves as primary trainer and support for Family Support Workers, including orientation training and ongoing performance support and monitoring.
Supports case conferencing with Family Support Workers as required.
Actively participates in the performance management of Family Support Workers, including selection, evaluation and corrective action as needed.
Ensures the rules and regulations of the NYS Office of Children and Family Services, Medicaid, and the Agency as they are applicable to direct services and ASFA guidelines for children and families, are adequately and appropriately adhered to.
Monitors and ensures compliance with required documentation including FASPs, progress notes, and reports.
Participates in County reviews and interdisciplinary team meetings.
Assists with policy development and continuous program process improvements.
Reviews and supports Willigent documentation to ensure compliance and accuracy.
Monitors foster home placements and ensures adherence to treatment plans.
Maintains regular contact with foster parents and youth.
Supports and supervises family visitation as appropriate.
Coordinates medical, behavioral health, and community services for each program participants.
Supports discharge planning and transition to community-based care as needed.
Serves as liaison with LDSS and community partners.
Reports critical incidents and responds appropriately to urgent concerns or crises as required.
Provides after-hours on-call coverage for the Therapeutic Foster Care Program (in program rotation).
Completes all FASPs and other supporting documents within the accepted time frame outlined by Standards.
Acts as a case planner, at County Request, relating to community agencies, coordinating services as needed to facilitate child and family reunification.
Oversees the services of the therapeutic foster home and ensures compliance with each family’s treatment plan through weekly contacts with foster parents and individual meetings with each child.
Convenes all case review conferences involving youth, parents, therapeutic foster parents and other service providers as needed.
Provides regular outreach family support and education sessions and refers family members for appropriate resources.
Participates on the interdisciplinary treatment team (CART) by attending weekly meetings and implementing treatment plans for children and families receiving services.
Maintains program statistical information as required by the Agency and all other funding sources.
Supervises and/or coaches visitation between family members as necessary to assure the safety and best interest of all members.
Provides transportation for children as necessary to fulfill the mandate of facilitating visitation between family members.
Attends and participates in in-service and foster parent trainings to develop and enhance social work skills.
Provides after-hours on-call coverage for the Therapeutic Foster Care Program (in program rotation).
Completes other duties as assigned.
Facilitates the exchange of health information and documentation of care received by the child in the community including attending any community-based care.
Coordinates Medicaid services that the child receives in the community and transition the child to home- or community-based living with outpatient treatment (e.g., individual and family therapy) as part of discharge planning.
If the child attends school in the community the staff coordinates care at school and delivered through the provider.
As needed, the Medical Escort accompanies the child to medical appointments and ensuring the child’s safety while under the Medical Escort’s supervision.
Participates in training that is commensurate with the intensity, scope, and duration of the needs of the children served.
