About The Position

The Senior Case Manager provides support to program managers, mental health professionals, and additional staff across the Kovler Center. Responsibilities include assisting the Manager of Case Management with organization of direct services and support to Kovler Center participants; facilitating coordination with other case managers throughout all KC programs, assisting in training new hires, volunteers and interns and participating in external meetings as well as with the different teams of clinical staff, program staff and leadership. Ensures record keeping about client visits, conducts regular meetings with clients and their families to provide support, answer questions and obtain general and clinical-related information, works with supervisors to develop strategies for team collaboration, and helps clients gain access to beneficial social programs, such as Medicaid, food stamps, etcetera. The Senior Case Manager accountabilities and work volume will be established by developing annual Success Objectives within the outlined framework. This Position is a Union Position.

Requirements

  • This position only requires an associate degree, although higher education may be valuable.
  • Prior experience as a Case Manager required; prior experience working with interns, volunteers and assisting in new staff training is preferred.
  • Second language required, French preferred.
  • Excellent oral and written communication skills.
  • Demonstrated competency in public speaking and strong interpersonal skills required.
  • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance.
  • Ability to add, subtract, multiply, and divide into all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

Responsibilities

  • Assist Manager of Case Management with the organization of direct services, preparing and conducting some trainings and shadowing opportunities for new staff/interns/volunteers, develop strategies for team collaboration and work productivity.
  • Work directly with intern(s) and help to determine interns duties and responsibilities, as well as provide training as necessary.
  • Build and maintain relationships with partner agencies and perform outreach for more resources and connections in the community.
  • Represent Case Management team and KC in some external partner meetings including information gathering opportunities regarding client services.
  • Facilitate internal meetings as warranted.
  • Keep accurate updates, organize records after every client visit to ensure that there are no undocumented breaks between treatments or visits.
  • Support applications for benefits assistance and answer questions about benefits along with additional resources available to individuals or refer them to the correct organization to get help.
  • Assist individuals in making referrals for various services including: legal, psychiatry, therapy, primary care, dental, Medical and Psychological Forensic Evaluations.
  • Meet weekly with supervisor to brainstorm and develop ways to improve team workflows and procedures, as well as determine better ways to serve participants.
  • Participate in-group activities daily and attend regular internal meetings with the team and external meetings as appropriate.
