About The Position
The Senior Case Manager will serve a caseload of approximately 20 clients, providing active case management for all assigned cases, including meeting weekly with each person individually. This role is part of the SOLAR program, a DHS funded Recuperative Care Program serving individuals experiencing homelessness with acute medical issues. The position requires a comprehensive understanding of community resources and the ability to advocate for clients' needs effectively.
