About The Position

The Senior Case Manager is responsible for ensuring that the highest quality support is afforded to all Day Habilitation members. The Senior Case Manager will also assist the Team Leader with extra assignments with a larger focus on guiding the day to day operations of the program. Are you ready to make a difference? Requirements

  • High School Diploma/GED
  • Bachelor's Degree in related field and 1 year of related experience or 2 years of related experience.
  • Must be able to perform each essential duty satisfactorily.
  • Must have ability to communicate effectively in writing.
  • Must have ability to read English and communicate effectively in the primary language of the program to which he/she is assigned.
  • Must be able to multi-task with frequently changing priorities.
  • Must possess basic computer knowledge.
  • Must have a demonstrated ability to work independently and as part of a larger resource team.
  • Must hold a valid drivers' license.
  • Must have access to an operational and insured vehicle and be willing to use it to transport consumers.

Responsibilities

  • Collaborate with the Team Leader to provide continuous quality improvement of individualized programs and services through the use of both internal and community resources.
  • Assist team members with the development of program curriculum designed to promote individual growth and self determination.
  • Develop and implement comprehensive written therapeutic service plans/individual support plans and implementation process for all assigned consumers.
  • Convey changes in consumer goals to the larger internal support team.
  • Maintain member records as required including timely and thorough completion of necessary program assessments, progress reports and other required documentation.
  • Compile review summaries for therapeutic service plans/individual support plans.
