About The Position

The Senior Case manager Specialty Behavioral Health provides high quality patient-centered care coordination as an integral part of a dynamic multi-interdisciplinary team. Care is provided in the clinic and outside in the community meeting each of our patients “where they are”.

Requirements

  • High school diploma or GED and at least 4year of full-time behavioral/integrated healthcare work experience.
  • Required to maintain a valid Arizona driver’s license and pass motor vehicle screening.
  • BLS Certification
  • Fingerprint clearance issued by Arizona Department of Public Safety required.
  • Must be at least 21 years of age.
  • Minimum of 1 year experience with behavioral/integrated health program service delivery PLUS
  • Minimum of 1 year court coordinator experience with patients on mental health Court ordered treatment.
  • Organization and care coordination
  • Communication and teamwork
  • Trained and able to work with HMIS.
  • Crisis intervention and de-escalation skills
  • Advocacy for client needs and service access
  • Knowledge of community resources and systems navigation, employment resources and VR referral processes.
  • Knowledge of substance use and/or mental health disorders
  • Trauma-informed care

  • Peer Support training and preferred
  • Experience in working with underserved populations is highly desirable.

Responsibilities

  • Conducting patient consultations, assessments, and care planning in collaboration with healthcare professionals.
  • Educating patients on conditions, treatment options, and available resources.
  • Coordinating services, referrals, and transitions of care, including housing, employment, and crisis interventions.
  • Monitoring patient progress, responding to changing needs, and facilitating re-engagement using trauma-informed approaches.
  • Monitoring Court ordered treatment and ensuring that agency and patient comply with mental health court order.
  • Advocating for patients and ensuring compliance with healthcare regulations and confidentiality standards.
  • Supporting hospital discharge planning and follow-up to reduce readmissions.
  • Completing documentation in the electronic medical record and managing prior authorizations.
  • Providing mentorship and onboarding support for new case management staff.
  • Serving as a point of contact for patients and advocates to resolve service-related issues.
  • Performing other duties as assignee
