About The Position

You will develop or contribute to personalized care plans and educate members and their families on available services and benefits. The role includes coordinating care, monitoring progress, providing psychosocial support, and collaborating with healthcare partners to ensure member needs are met. You may also perform on-site visits and serve as a Subject Matter Expert (SME) to support the care navigation team.
