About The Position

The Senior Care Manager provides leadership and supervision to a team of Care Managers, each responsible for a portfolio of approximately 50 clients. Reporting to the Director of Operations (or equivalent senior leader), the Senior Care Manager ensures high-quality service delivery, compliance with care standards, and consistency in client outcomes. This role is pivotal in mentoring Care Managers, monitoring caseload performance, and fostering strong collaboration across teams and community partners.

Responsibilities

  • Supervise a team of Care Managers, providing guidance, mentorship, and performance feedback.
  • Conduct regular check-ins, case reviews, and team huddles to ensure alignment with organizational standards.
  • Provide real-time support for escalated client concerns, complex cases, or crisis situations.
  • Foster a culture of accountability, empathy, and continuous improvement.
  • Monitor Care Manager caseloads (50 clients per CM) to ensure balanced workloads and quality of care.
  • Review assessments, care plans, and documentation for accuracy, completeness, and compliance.
  • Track key performance indicators, including client satisfaction, service utilization, and program outcomes.
  • Identify trends or gaps in services and implement corrective strategies.
  • Support Care Managers in resolving high-need or complex client cases.
  • Act as an escalation point for advocacy, ensuring clients receive appropriate and timely resources.
  • Provide oversight of client care plans, making adjustments as needed to improve effectiveness.
  • Onboard and orient new Care Managers, ensuring they understand expectations, protocols, and best practices.
  • Conduct quarterly and annual performance reviews for Care Managers, setting clear goals for professional growth.
  • Provide coaching in assessment techniques, client engagement, documentation, and resource coordination.
  • Facilitate training sessions and encourage ongoing professional development.
  • Ensure that Care Managers maintain accurate, timely, and compliant documentation.
  • Generate team-level reports on client outcomes, caseload productivity, and program effectiveness.
  • Support audits and reviews by preparing and submitting required documentation.
  • Ensure compliance with Medi-Cal, CalAIM, and organizational standards.
  • Build and maintain relationships with community organizations, health plans, and referral partners.
  • Represent the Care Manager team in interdepartmental meetings and external collaborations.
  • Work with leadership to refine workflows, improve cross-team communication, and expand available resources.
