About The Position

Heritage Health Network (HHN) is looking for a Senior Associate, Growth & Business Development to drive expansion of our Enhanced Care Management (ECM) program through strategic healthcare partnerships. This is a high-impact, field-based role where you’ll work directly with PCPs, IPAs, hospitals, ED teams, and behavioral health providers to generate referrals, build long-term partnerships, and grow membership across our health plan network.

Requirements

  • 3+ years in healthcare business development, partnerships, or provider-facing sales
  • Experience working with physician practices, hospitals, or health systems
  • Proven ability to drive growth, referrals, or revenue outcomes
  • Strong consultative selling and relationship-building skills

Nice To Haves

  • Familiarity with Medi-Cal, CalAIM/ECM, HEDIS, or value-based care (preferred)

Responsibilities

  • Build and manage relationships with clinical and community partners to drive referral pipelines
  • Conduct regular in-person outreach and represent HHN in the field
  • Identify and close new partnership opportunities, including executing MOUs
  • Educate partners on how HHN improves P4P performance, HEDIS measures, and quality outcomes
  • Own and manage a CRM pipeline, tracking referrals through enrollment
  • Partner cross-functionally with Operations and Care Management to ensure strong partner experience
