About The Position

The Salvation Army is dedicated to Doing the Most Good, comprising thousands of officers, soldiers, staff, volunteers, donors, and partners working together to meet the physical, emotional, and spiritual needs of vulnerable neighbors. The Senior and In-Home Services Case Manager will provide emotional support to seniors aged 60 and older in crisis or at-risk situations, helping them stabilize through referrals and resources, and assisting them in identifying services to maintain independent living.

Requirements

  • Minimum of a BA or BS with a MA, MS or MSW preferred.
  • Experience working with seniors and families.
  • Driver’s license, MVR clearance, and a working, appropriately insured automobile.

Responsibilities

  • Conduct in-home assessments.
  • Observe client home environment and interaction with family members/neighbors.
  • Record observations of home condition, family behaviors, and relationships in readable and accurate 'Case Notes' form.
  • Maintain contact with Chicago Department of Family and Support Services/Senior Services to coordinate client services.
  • Work collaboratively with partners and develop new partnerships within The Salvation Army and the community.
  • Maintain timely documentation of participant’s involvement including assessments, demographic data, case notes, closing summaries, and outcomes data collection.
  • Identify community resources and encourage clients to utilize them.
  • Act as a liaison between client and various organizations.
  • Enter all information into the city database in a timely fashion.
  • Obtain appropriate releases of information.
  • Conduct outreach activities to raise awareness and participation in the program.
  • Initiate contact with the Illinois Department of Children and Family Services as a mandated reporter of suspected child abuse or neglect.
  • Initiate contact with the Elder Abuse Hotline as a mandated reporter of suspected elder abuse.
  • Participate in weekly supervision and case consultation with the Program Manager.
  • Attend Senior & In-Home Services staff meetings as scheduled.
  • Maintain awareness of applicable liability and legal issues.
  • Abide by ethical and culturally competent standards as outlined by the National Association of Social Workers and The Salvation Army.
  • Perform other duties as assigned.
