Requirements

  • Bachelor’s degree in marketing, communications, or related field or equivalent experience
  • At least 2 years of hospice or home health sales experience with a proven record of growth
  • Strong relationship-building, communication, and presentation skills
  • Ability to work independently while collaborating effectively with cross-functional teams
  • Comfort engaging with patients, families, and healthcare professionals
  • Valid Colorado driver’s license and reliable transportation (meeting driver standards)
  • Ability to meet immunization and health screening requirements

Responsibilities

  • Own and grow referral relationships within your assigned territory, including physicians, facilities, care providers, and community agencies
  • Develop, implement, and evaluate quarterly and annual territory growth plans
  • Build strong partnerships through effective sales calls, presentations, and education
  • Serve as a key liaison between referral partners and internal teams to ensure seamless communication and service delivery
  • Meet with patients and families to discuss home health, hospice, and palliative care services and manage expectations
  • Admit patients to hospice services when appropriate
  • Collaborate closely with admissions, clinical teams, and leadership to support referral success
  • Track activity, opportunities, and follow-ups in CRM; analyze market and competitor data
  • Represent the organization at physician offices, lunch-and-learns, health fairs, CEU programs, and community events
  • Assist in developing sales collateral, marketing content, and territory strategies
  • Prepare monthly referral and market tracking reports
  • Embrace teamwork, accountability, and continuous improvement
