About The Position

The SHCH DSP works independently one-on-one in self-directed community habilitation assisting the individual receiving services in meeting his/her goals as identified in their service plan. The SHCH DSP is an important link between the individual, the broker, the community, and the circle of support acting as both a provider of direct support services and as an advocate promoting person centered services and community inclusion. The SHCH DSP depending on the plan, may provide services that take place in the home, community, or both. The SHCH DSP carries out all duties according to Agency, Program and NYS OPWDD policies and regulations.

Requirements

  • Safeguard the individual and follow goals and safeguards identified in the plan.
  • Must have the ability to work with the family and/or individual
  • Demonstrates OPWDD Core Competencies and follow OPWDD Code of Ethics.
  • Must have the ability to work with others in a warm, professional manner and be a positive role model to individuals.
  • Must have the ability to read and comprehend instructions, correspondence, and have the ability to write simple correspondence.
  • Must have basic phone app/computer skills to adequately document notes within Evero Software, email etc.
  • Compliance with documentation and other requirements daily.
  • Must have basic reasoning skills, including the ability to analyze and problem solve.
  • Must possess a valid New York State driver's license with good driving record in accordance with Agency policy and procedure and have access to a reliable and insured vehicle.
  • Must have basic computer skills to adequately document notes in Evero software in addition to accurately documenting timesheets, mileage etc.
  • Must be able to tolerate a moderate noise level in the work environment, handle distractions, while performing this job may be required to sit, stand, use hands, reach, talk, hear, walk or lift up to 50 pounds.
  • Within 30 days of hire: you must successfully complete all Agency and program required orientation/trainings.
  • Must maintain all required certifications and trainings and attend additional trainings as required for skill enhancement.
  • High School Diploma or G.E.D preferred, applicable job experience preferred.
  • 1-3 years related paid or life experience with special populations.

Responsibilities

  • Provides person centered support to individuals and adheres to OPWDD core competency expectations for a DSP.
  • Follow guidelines and instructions given by the Broker and the individual/circle of support to meet the individual’s specific support needs.
  • Teaching skills needed to meet the individual’s goals as identified in the individual’s plan.
  • Provide documentation to assist with evaluating service goals.
  • Follows safeguards identified in the plan.
  • Participates in and implements Staff Action Plan; submit daily documentation consistent with Medicaid compliance using Evero software, reading email etc.
  • Make recommendations as well as offer ideas and suggestions to increase community inclusion.
  • Provides safe and reliable transportation.
  • Follow health and safety practices always including universal precautions.
  • Documents and reports incidents in accordance with OPWDD regulations and Agency procedures.
  • Maintains paperwork as required by state and program regulations and agency policy in an accurate and timely manner with attention to detail.
  • Maintains confidentiality of all information.
  • Demonstrates OPWDD Core Competencies and follows OPWDD Code of Ethics.
  • Follow all policies and procedures of the organization as applicable.
  • Participate in required orientation and annual training.
  • Maintain safe driving practices and submit updated license and driving insurance information throughout employment.
  • DSP duties will be specific to the Individual including but not limited to personal care, supervision, protective oversight etc.
  • Provide face to face services which is implemented through home visits and community activities.
