About The Position

As a Self-Directed Services Plan Coordinator you will be responsible for obtaining, tracking and verifying accuracy of all necessary plans, certifications, and agreements for people who self-direct their services for both regulatory and Agency requirements.

Requirements

  • High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Valid NYS driver's license required
  • Required to lift up to 5 pounds and may occasionally be required to lift up to 10 pounds.
  • Excellent time allocation, multitasking and personal organization skills; basic computer skills
  • Needs to be comfortable working in a variety of settings: Work is generally performed in an office setting with some hours in a variety of community settings for meetings including home environments (contact with children and pets; exposure to seasonal weather during travel to meetings, conferences and trainings)

Responsibilities

  • Attend Life Plan and other meetings held by a person who self directs and their Circle of Support.
  • Take notes and follow up with members of the team as needed to obtain Life Plans, Staff Action Plans, and all other necessary documents.
  • Review all plans and documents for accuracy based on your notes and your understanding of the wishes of the person supported.
  • Work with the Support Broker and Care Manager if there are questions or missing information.
  • Upload all necessary plans and documentation into the eVero online platform.
  • Daily review of the eVero platform to check the status of all required information and documents, follow up with appropriate party on anything that is missing or past due.
  • Daily review of Medicaid billing documentation to ensure accuracy and relevance to the established goals in the Staff Action Plan/Life Plan.
  • Ensure compliance with all OPWDD guidance, administrative memorandums (ADM's) and other applicable state regulations as well as Agency policies and procedures
  • Ensure that all records are accurate and readily available for annual Corporate Compliance reviews
  • Ensure all electronic files are organized and current
