About The Position

The Care Manager - Nursing (RN CM) at Point32Health is responsible for ensuring that members receive comprehensive care management across various healthcare settings. This role involves coordinating care transitions, managing complex cases, and implementing wellness interventions to empower members towards optimal health. The RN CM collaborates closely with members, caregivers, and healthcare providers to develop and evaluate personalized care plans, focusing on improving health outcomes and self-management skills.

Requirements

  • Registered Nurse with current unrestricted license in state of residence.
  • Bachelor's Degree in Nursing preferred.
  • 5+ years of relevant clinical experience.
  • Experience in home care or case management preferred.
  • Proficiency in a second language desirable.
  • Experience in specialty areas such as oncology, neurology, or chronic condition management is a plus.

Nice To Haves

  • National certification in Case Management desirable.

Responsibilities

  • Administer assessments and collaborate with members/caregivers and providers to develop a plan of care.
  • Implement member-specific care management interventions and evaluate the plan of care, revising as needed.
  • Facilitate program enrollment utilizing motivational interviewing skills.
  • Provide targeted health education and proactive strategies for condition management.
  • Perform telephonic and face-to-face outreach to assess barriers to wellness and member needs.
  • Coordinate safe transitions to the next level of care, ensuring understanding of post-hospital discharge instructions.
  • Document case details in the applicable care management system according to standards.
  • Collaborate with the interdisciplinary care team to improve member outcomes.
  • Attend and present high-risk members at interdisciplinary rounds forum.
  • Maintain professional growth and development through self-directed learning activities.
