About The Position

The Children’s Center of Hamden is seeking a School Social Worker to provide clinical services and case management as part of a multidisciplinary treatment team in our Whitney Hall School program. In this role, you’ll support youth and families by addressing non-academic barriers to learning, providing counseling and crisis intervention, and coordinating care with school districts and community partners. Requirements

  • Holds or is immediately eligible to obtain Connecticut License Master of Social Work (LMSW) with the State of Connecticut
  • Holds or is immediately eligible to obtain Social Worker Certification (#071) with the Connecticut Department of Education
  • Strong documentation, communication, and collaboration skills
  • Comfortable responding to urgent situations and supporting crisis needs when they arise

Responsibilities

  • Provide individual counseling and social-emotional support aligned with IEP goals
  • Complete assessments, crisis intervention/risk assessments, and safety planning as needed
  • Support PPT/IEP processes and help develop behavioral and social-emotional plans (FBA/BIP when appropriate)
  • Collaborate with teachers, related services, and the treatment team to support student progress
  • Coordinate with families, school districts, and outside agencies from admission through discharge
  • Perform other duties as assigned to support program success.
