Requirements

  • In-depth understanding of case management and rehabilitation principles for mental health and/or substance use disorders.
  • Familiarity with community resources and the ability to connect clients to appropriate services.
  • Knowledge of evidence-based practices, legal requirements, and court procedures related to mental health and substance use treatment.
  • Strong interpersonal and communication skills, both verbal and written, with the ability to produce clear and concise documentation.
  • Self-motivated with the ability to balance multiple priorities and maintain a high standard of service.
  • Proven ability to establish effective relationships with clients, families, and professional teams.
  • For ACT/ACT TAY/IMPART teams: Demonstrate understanding of the Assertive Community Treatment model, including its core processes, interdisciplinary collaboration, and service delivery.
  • Bachelor’s degree in Psychology, Social Work, or a related human services field; or Bachelor’s degree in an unrelated field with two years of relevant experience; or Any combination of four years of higher education and related experience; or Associate of Applied Science in Behavioral Health Support; or Four years of qualifying experience delivering services to individuals with mental health disorders, substance use disorders, or developmental disabilities.
  • Successful completion of background check including criminal record, driving record, abuse/neglect and fingerprint check.
  • Completion of New Hire Orientation at the beginning of employment.
  • All training requirements including Relias at the beginning of employment and annually thereafter.
  • Current driver’s license, acceptable driving record and current auto insurance.
  • Must have reliable means of transportation to transport clients in personal vehicle.
  • ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (exists up the 1/3 of the time) and/or a negligible amount of force frequently (exists 1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body . Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift. Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time . Jobs are sedentary if walking and standing are only occasionally, and all other sedentary criteria are met.

Responsibilities

  • Provide community-based mental health services to individuals across the lifespan, including assessments, treatment planning, and intervention implementation.
  • Collaborate with clients, families, and multidisciplinary teams to create and deliver individualized care plans that emphasize empowerment, resilience, and personal growth.
  • Utilize evidence-based practices such as Motivational Interviewing, Trauma-Informed Care, Positive Reinforcement Techniques, and skills rooted in CBT and DBT frameworks.
  • Offer behavioral interventions, including cognitive processing, group dynamics, and operant conditioning, tailored to client needs.
  • Coordinate care with external providers such as primary care physicians, psychiatrists, educators, and local organizations to ensure a holistic approach to client support.
  • Facilitate and lead group sessions related to psycho-social rehabilitation (PSR), based on client treatment goals.
  • Maintain accurate and timely documentation of client interactions, progress, and treatment outcomes, adhering to organizational policies and legal requirements.
  • Participate in professional development opportunities, staying current on best practices in mental health care and evidence-based techniques.
  • Actively contribute to quality improvement initiatives through data collection, reporting, and collaborative efforts.
  • Develop and maintain strong relationships with community stakeholders, including families, educators, and other key influencers in clients' support systems.
  • Respond effectively to client crises, ensuring appropriate resources and interventions are in place.
