For additional questions please contact us at: [email protected]. Job Role SummaryThe Case Manager (Tier I) provides intensive, school-based case management for pregnant and parenting teens. Responsibilities include comprehensive assessments, individualized care plans, benefit enrollment/navigation (WIC, Medicaid, CCDF), warm referrals, group facilitation, documentation in Insight, and progress monitoring across academic, behavioral, and social-determinant goals. The role collaborates with families, teachers, counselors, healthcare providers, and community partners to reduce barriers and improve outcomes.Essential DutiesStudent Case Management & Care Planning (50%) · Conduct strengths-based assessments; co-create care plans with short/long-term goals; document in Insight.· Monitor academic attendance/performance and coordinate interventions with school teams.· Provide benefits navigation (WIC, Medicaid, CCDF) and connect to community resources; complete follow-ups. Health Education, Group Work & Family Engagement (20%) · Deliver evidence-based lessons (parenting, MCH topics); facilitate small groups.· Engage parents/guardians; coordinate meetings with teachers, counselors, therapists. Coordination & Referrals (15%) ·Serve as liaison with clinics, social service agencies, and community-based organizations; ensure warm hand-offs and closed-loop referrals.· Conduct home or hospital visits when necessary; maintain safety protocols. Data, Reporting & Quality (10%) · Maintain timely, accurate EMR documentation; track outcomes (attendance, credit accrual, benefit uptake).  Submit weekly schedules and reports· Support grant/program reporting and basic quality-improvement activities. Professionalism & Compliance (5%) · Uphold HIPAA/FERPA and mandated-reporting requirements; complete required trainings.· Represent the program positively with students, families, schools, and partners.Associated Job Duties· Assist with incentive stores, recognition events, college tours, and community service projects.· Participate in evaluation activities and staff/team meetings; present as requested at local events.· Other duties as assigned.

Requirements

  • Bachelor’s degree in Social Work, Public Health, Human Services, Education, Psychology, or related field.
  • 1–3 years case management experience working with at-risk youth/families in school, community, or home settings (internships count).
  • Familiarity with benefits navigation (WIC/Medicaid/CCDF) and community resource mapping.
  • Indiana CHW, Family Development Credential (FDC), Wraparound, Motivational Interviewing, CPR/First Aid.
  • Valid Indiana Driver’s License and current auto insurance required.
  • Adolescent development, Title IX basics, Positive Youth Development, Trauma-Informed Practice.
  • Strong documentation in EMR/case systems (Insight), privacy (HIPAA/FERPA), and time management.
  • Clear, empathetic communication; culturally responsive practice.

  • Bilingual (Spanish) preferred.
