About the position

The School-Based Site Coordinator at Affinity Health Center is responsible for supporting multiple school-based sites in York County, acting as a champion for primary care and mental health care for children, youth, and their families. This role involves collaboration with school district staff, teachers, counselors, nurses, and parents/guardians to enhance access to medical and mental health services. The coordinator will oversee program development, implementation, quality assurance, and evaluation of care at the school sites, ensuring they are well-resourced and promoting positive patient experiences.

Responsibilities

• Ensure collaboration and engagement with school district staff, teachers, counselors, nurses, and parents/guardians.
• Increase access to medical care and mental health services for students and their families.
• Support program development, planning, implementation, quality assurance, and evaluation of care at school-based sites.
• Promote school-based sites within the school communities and the community-at-large.
• Champion patient-centered care with compassion, dignity, and respect.

Requirements

• High School Diploma or equivalent required.
• Valid Driver's License and dependable transportation required.
• Relevant experience in community engagement, communication, and coordination of services preferred.
• College degree preferred.
• Bi-lingual (English and Spanish) preferred.
