Summary: School-Based Health (SBH) requires flexible, efficient teamwork on the part of the provider and other clinical staff. The provider and nurse work together to perform all the needed duties for clinic maintenance and patient care in School Based Clinics. ESSENTIAL DUTIES AND RESPONSIBILITIES: Obtain and maintain records of parental consent, insurance information, and patient history for students and staff of SBH. Coordinate with school secretaries and health department nurses to appropriately screen and triage potential patients for SBH. Maintain safe and well-equipped clinical areas at all SBH sites. Duties include, but are not limited to inventory, ordering, and stocking medications and supplies, reporting malfunctioning equipment or facilities, and appropriate security/storage of supplies. Have knowledge of and oversee compliance with OSHA and on-site licensure requirements for SBH. Perform duties of Front Office Personnel, i.e., check in, appointment scheduling, follow-up monitoring, insurance verification, and check out as needed. Schedule outpatient x-rays, labs, and referrals and fax orders to appropriate facilities. Obtain results of ordered studies for follow up and interpretation by provider. Assist with follow up phone calls or other needed communication to the parent/guardian of SBH patients daily. Participate in school functions as appropriate for SBH, i.e., Open House, school registration, and special events. Assess patients’ emotional and personal needs, i.e., food, shelter, clothing, abuse, neglect and refer to appropriate resource agencies. Perform teaching as needed to patients and parents related to health maintenance, disease prevention, and care for illnesses or injuries. Perform initial assessment of age, weight, vital signs, and chief complaint and document in medical record. Properly prepare the patient for the exam and assist the provider as needed with exams and procedures. Make certain that all pertinent pre-clinic laboratory or other pertinent data is in chart at time of visit. Always practice universal precautions utilizing available safety equipment (gloves, shields, etc.) Maintain well-cleaned and stocked work areas. Clean and sanitize medical equipment regularly. Have computer knowledge of the EHR and be proficient in clinical workflows. Complete and document required patient care activities to comply with all regulatory measures (HEDIS, UDS, PCMH, and FQHC). Prepare and administer immunizations, allergy, or other injections as needed. Assist with lifting patients as needed. Obtain patient insurance pre-authorization for all referrals and procedures. All team members will be involved in the process of improving quality incomes. Team members will participate in the review and evaluation processes of practice performance and help to identify opportunities for improvement. Team members will participate in Grace Health's advocacy program. GENERAL DUTIES: Follow policies and procedures of the office, including administrative, clinical, quality assurance, and personnel. Maintain good attendance (daily, meetings, and other assignment tasks). Maintain timely documentation of all work assignments. Maintain patient confidentiality. Routinely keep supervisor informed about attendance and job assignments. Flexible in being able to multitask. Work effectively and at an efficient pace. Work cooperatively with providers, administration, and peers.
