About The Position

The Center for Human Development, (CHD) is seeking a School-Based Community Support Worker to join our Community School Based Clinic based in Franklin County. Your role as a School-Based Community Support Worker: The School Based Tele Behavioral Health Program provides students with year-round long term or short-term counseling via tele and community resources. The Community Health Worker will work alongside a Clinician and school personnel to help students and families access services and resources. They will provide case management; ensuring students and families receive the help they need. They will document encounters; collect data and complete monthly reports. They will support with making referrals, obtaining releases of information, consents and other medical documentation. Our Community Support Team provides outreach and support services to clients with a behavioral health diagnosis to enable them to utilize clinical treatment services and other supports. The Case Manager assists clients to attain his/her clinical treatment plan goals in outpatient and other levels of care and works to mitigate barriers to success.

Requirements

  • Bachelor's degree in a human services related field from an accredited educational institution
  • Strong computer skills- Electronic medical record (EMR) databases
  • Knowledge of area community agencies and resources
  • Skill in organizing resources and establishing priorities
  • Creative and analytical thinking
  • Strong interpersonal and communication skills

  • Masters of Science or better

Responsibilities

  • Provide home, school and community-based care coordination to youth and their families
  • Assist students and families to access on site tele behavioral counseling services as well as offsite community-based clinicians and non-clinical resources (as needed)
  • Conduct comprehensive screening/assessment activities including mental health status, health screen and social needs factors, to support community-based referrals
  • Provide ongoing follow-up, motivational interviewing and goal setting with students/families
  • Provide service coordination and linkages to the community
  • Provide outreach services to clients, delivering mobile services in a number of community or institutional settings including inpatient units, homes, day programs, etc.
  • Assist clients in obtaining benefits, housing and health care as needed
  • Provide crisis support and participate in the development, revision and utilization of crisis prevention and/or safety plans
  • Encourages and facilitates utilization of clients' natural support systems as well as recovery-oriented, peer support and self-help support and services
