School-Based Community Support Worker
About The Position
The Center for Human Development, (CHD) is seeking a part time School-Based Community Support Worker to join our Community School Based Clinic based in Franklin County. Your role as a School-Based Community Support Worker : The School Based Tele Behavioral Health Program provides students with year-round long term or short-term counseling via tele and community resources The Community Health Worker will work alongside a Clinician and school personnel to help students and families access services and resources. They will provide case management; ensuring students and families receive the help they need. They will document encounters; collect data and complete monthly reports. They will support with making referrals, obtaining releases of information, consents and other medical documentation.
Requirements
- High School Diploma or Equivalent required
- Strong computer skills- Electronic medical record (EMR) databases
- Knowledge of area community agencies and resources
- Skill in organizing resources and establishing priorities
- Creative and analytical thinking
- Strong interpersonal and communication skills
Responsibilities
- Provide home, school and community-based care coordination to youth and their families
- Assist students and families to access on site tele behavioral counseling services as well as offsite community-based clinicians and non-clinical resources (as needed)
- Conduct comprehensive screening/assessment activities including mental health status, health screen and social needs factors, to support community-based referrals
- Provide ongoing follow-up, motivational interviewing and goal setting with students/families
