About The Position

The School Based Case Manager at Children’s Advantage is responsible for providing solution-focused interventions, skills building, and behavior management strategies in both school and home settings. This role involves coordination and assistance in crisis intervention, ensuring that the needs of the students are met effectively.

Requirements

  • Ability to learn about community resources.
  • Ability to effectively communicate with multi-disciplines associated with community resources.
  • Effective time management and interviewing skills.
  • Awareness of the service population’s cultural and socio-economic characteristics.
  • Valid Ohio Driver’s License and ability to meet agency requirements for driving insurability.
  • Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Identification & Investigation) if applicable.

Responsibilities

  • Interview and assess the needs of the persons to be served via strength-based identification.
  • Provide solution-focused interventions, skills building, and behavior management strategies in school and home settings.
  • Network with other professionals contributing to the students’ mental health needs.
  • Conduct service coordination meetings with school staff, guardians, students, and other community members.
  • Monitor the overall delivery of provided services and note progress.
  • Provide advocacy, training, and facilitate linkage for the persons served in the use of basic community resources and social support networks.
  • Provide coordination and assistance in crisis intervention and stabilization to persons served with availability on a 24-hour basis or refer to access for non-business hours.
  • Attend and participate in staff & treatment meetings, workshops, and supervision.
  • Track designated outcomes.
  • Adhere to agency personnel policy and procedures, ODMHAS standards, and fulfill documentation and productivity requirements.
  • Comply with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
  • Perform other duties as assigned.

Benefits

  • $39,000/year salary.
  • Full-Time, 40 hours/week schedule.
