The Scheduling Coordinator at Home Care Connectors LLC is responsible for effectively matching caregivers with clients while managing ongoing patient needs. The role emphasizes continuity in care and requires addressing employee issues to maintain departmental standards.

  • Develops new processes to improve workflow and efficiency and reinforces compliance with existing processes.
  • Manage weekend schedule including phone inquiries, callouts, and patient/caregiver requests.
  • Receives all complaints/problems related to off-hour issues or on-call.
  • Schedules caregivers to assist clients in a timely manner.
  • Notifies the appropriate and responsible staff of caregivers' emergencies, sickness, and other imminent occurrences that may affect the client.
  • Ensure compliance for caregivers' written time off requests 2 weeks or more in advance for planned time off.
  • Actively participate in caregiver orientation program.
  • Lead point of contact for clients and caregivers requesting home care.
  • Work with care managers to ensure client caregiving needs are met.
  • Participate actively with new client intake process.
  • Communicate schedule changes to clients and family members.
  • Task client care manager via Clearcare.
  • Process other duties as assigned.
  • A minimum of three (3) years recent experience as a scheduling coordinator in a home care agency.
  • Excellent customer service and communication skills to provide scheduling coordination for homemakers and companions.
  • Ability to multi-task and a propensity for working with and helping people.
  • Must have a criminal background check.
