About The Position

Scheduling Coordinator- The heartbeat behind the care. If you love puzzles, people, and making things run smoothly, this might be your perfect role. Our Home Health Scheduling Coordinator is the behind-the-scenes hero who makes sure the right clinician gets to the right patient at the right time, every time. You are the operational hub that connects clinicians, patients, leadership, and systems to ensure care is delivered reliably, compassionately, and in compliance with all regulations. You will be part air-traffic controller, part problem-solver, part customer service pro, and 100 percent essential to our mission of delivering exceptional home health care. What You’ll Be Doing Scheduling & Coordination You will build and manage daily clinician schedules so patient visits happen on time and according to physician orders and agency policies. You will adjust schedules when life happens, like call-outs, urgent needs, and census changes, to keep care uninterrupted. You will thoughtfully balance clinician availability, skill sets, geography, and productivity expectations. Systems & Documentation You will keep our scheduling and EMR systems accurate and up to date. You will run daily and weekly reports and follow up on outstanding items to support timely care. You will clearly communicate schedule updates to clinicians, leadership, and other teams. Productivity & Incentives Management You will monitor visit completion, clinician productivity, and schedule efficiency to support operational goals. You will help track and coordinate eligibility for extra visit pay, incentives, and productivity bonuses according to company guidelines. You will flag trends, gaps, or opportunities to leadership to improve visit coverage and performance. Communication & Customer Experience You will be a go-to contact for clinicians when scheduling questions or changes come up. You will communicate kindly and professionally with patients, families, and referral partners as needed. Operational Support You will help orient new clinicians to scheduling workflows and expectations. You will participate in after-hours or weekend on-call rotation when needed to support patient care. Compliance & Confidentiality You will protect patient privacy and follow HIPAA and all regulatory requirements. You will operate within all federal, state, and agency policies and compliance standards.

Requirements

  • A high school diploma or equivalent is required.
  • Comfort using electronic medical records and standard computer systems.
  • Basic knowledge of medical terminology and home health workflows.
  • Strong organization, time management, and problem-solving skills.
  • Excellent communication skills and a friendly, professional phone presence.
  • Ability to work independently, think on your feet, and juggle priorities in a fast-paced environment.

Nice To Haves

  • Healthcare-related education is a plus.
  • Experience in home health scheduling or healthcare coordination is strongly preferred.
