About The Position

Amerita is a leading provider of Specialty Infusion services focused on providing complex pharmaceutical products and clinical services to patients outside of the hospital. The Nursing Coordinator/scheduler has primary responsibility for scheduling new nursing and ongoing visits and distributing work flow in the department to assure that both new nursing visits and ongoing nursing visits are completed timely and within start of care and patient time expectations. This is an in-office position.

Requirements

• High School Diploma/GED or equivalent.
• Basic knowledge and experience in home care preferred.
• Good organizational and time management skills.
• Excellent verbal and written communication skills.

Responsibilities

• Coordinates and tracks regularly scheduled Amerita nursing visits for field and AIS.
• Secures Home Health agency for patient visits when Amerita nurse is unable to provide nursing.
• Communicates patient demos and most recent infusion orders to field nurses.
• Alerts field nurses when there is a change or an update to an infusion order.
• Communicates assignment of new patients and primary nursing from Nurse Manager to field staff.
• Confirms authorizations and reauthorizations for nursing visits are secured as needed.
• Provides infusion treatment plans to treating providers and maintains communication log.
• Tracks outside agency visits by month.
• Contacts outside agencies for nursing notes, statements and invoices as needed.
• Maintains files of credentials and insurance documents for outside agencies.
• Collaborates with Intake, Sales and Pharmacy staff to schedule visits and orders.
• Reviews and approves nursing notes in Alayacare or applicable documentation software.
• Reviews and approves subcontract billing.
• Triages calls from patients, providers and internal partners regarding scheduled nursing visits.
• Assigns patient risk codes for all new patients.
• Assists branch with weekend on call duty.
