SAGE , the specialized memory care division of Seniors Home Care , is the leading provider of person-centered, activity-based dementia support in the St. Louis and South County area . Requirements

  • Active CNA license
  • Minimum 3 years of professional experience in dementia or memory care
  • Ability to lift at least 35 lbs.
  • Valid driver's license, reliable vehicle, current registration and insurance
  • Must pass a background check
  • Proof of eligibility to work in the U.S.

Responsibilities

  • Companionship & Engagement: Foster connection through conversation, games, music, and mentally stimulating activities tailored to the individual.
  • Daily Living Assistance: Support clients with personal care needs such as bathing, dressing, toileting, mobility, grooming, and feeding.
  • Light Housekeeping & Errands: Assist with meal prep, laundry, cleaning, and transportation as outlined in the care plan.
  • Monitoring & Communication: Observe and report changes in client condition, behavior, or cognition to the Care Coordination Team.
  • Complete documentation after each shift.
