SAGE , the specialized memory care division of Seniors Home Care , is the leading provider of person-centered, activity-based dementia support in the St. Louis and South County area . Requirements
- Active CNA license
- Minimum 3 years of professional experience in dementia or memory care
- Ability to lift at least 35 lbs.
- Valid driver's license, reliable vehicle, current registration and insurance
- Must pass a background check
- Proof of eligibility to work in the U.S.
Responsibilities
- Companionship & Engagement: Foster connection through conversation, games, music, and mentally stimulating activities tailored to the individual.
- Daily Living Assistance: Support clients with personal care needs such as bathing, dressing, toileting, mobility, grooming, and feeding.
- Light Housekeeping & Errands: Assist with meal prep, laundry, cleaning, and transportation as outlined in the care plan.
- Monitoring & Communication: Observe and report changes in client condition, behavior, or cognition to the Care Coordination Team.
- Complete documentation after each shift.
