About The Position

Under the direction of the Director of Social Services and the CQI RN, the Ryan White LPN Data Nurse is responsible for coordinating and executing all Ryan White data and clinical quality management (CQM) activities. The responsibilities of the LPN Data Nurse include generating and reconciling all Ryan White Program data and CQM reports and data collection in EHR, Practice Management System, CAREWare and/or other appropriate data entry and reporting systems. The LPN Data Nurse maintains accurate data collection and reporting in accordance with federal requirements. In addition, the LPN Data Nurse participates actively in Performance Improvement activities, such as Plan, Do, Study, Act (PDSA) and tracks and trends Ryan White Programs’ performance measures.

Requirements

  • Associate’s Degree in Nursing.
  • Must have knowledge of social services and health care delivery and data systems.
  • At least three years of experience in community health or health education related setting.
  • Valid license as a Licensed Practical Nurse (LPN) in Mississippi required.
  • Valid Mississippi Driver’s license required.
  • Current Basic Life Support (BLS) through American Heart Association.
  • Must be able to continuously use sound communication and counseling skills in coordinating RWP data activities.
  • Must be able to reason in planning and reporting.
  • Must be able to communicate effectively both orally and in writing.
  • Must be able to deal effectively with patients, staff and other health professionals.
  • Must be able to pass a criminal background check, drug screen and physical assessment.
  • Must be willing to do local and out-of-state travel
  • Must be able to sit, stand, or walk for long periods of time
  • Must be able to lift a minimum of 10 lbs. routinely
  • Must continuously listen, visualize, and have dexterity and eye-hand coordination
  • Ability for simple grasping and speed work
  • Must frequently sit, squat, reach, use both right and left forearm rotation, walk and climb stairs
  • Must occasionally kneel, twist, have ability to grasp firmly, lift and carry, push and pull in excess of 10 lbs.

Responsibilities

  • Demonstrates the ability to coordinate and execute all Ryan White data activities
  • Understands the goals and objectives of the Corporation as set out in the By-Laws and Policy and Procedures Manuals
  • Understands the goals, clinical guidelines and performance measures of the Ryan White programs
  • Works closely with the Director of Social Services in reviewing and updating program services and plans
  • Assists with the grant application process by generating accurate data and clinical quality management reports
  • Participates in meetings of community groups, local organizations, professional groups and regulatory agencies to promote the program
  • Establishes linkages with public and private clinical care referral sources
  • Participates actively in Performance Improvement activities, such as Plan, Do, Study, Act (PDSA) and tracks and trends RWP performance measures
  • Maintains accurate data collection and reporting in accordance with federal requirements.
  • Generates and reconciles the daily schedule of Ryan White patients with completed appointments and encounter reports in the EHR, Practice Management System, CAREWare and/or other appropriate data entry and reporting systems
  • Generates reports to verify and reconcile required Ryan White Program’s services in CAREWare by the medical case managers and other RWP staff
  • Attends meetings of the Ryan White Programs Network, the SAETC trainings, and other appropriate meetings
  • Reports at least monthly to the Director of Social Services regarding the Ryan White program data activities, and explains, clarifies, or interprets CQM issues which may affect data reporting/results
  • Documents all patient encounters, including Rapid HIV testing, within 24 hours of the date of service in EHR and CAREWare and generates an encounter for all face-to-face patient encounters on the date of service
  • Participates in meetings with the Ryan White Program staff and the social services staff to ensure coordination
  • Demonstrates positive personal attributes such as professionalism, honesty, and integrity
  • Demonstrates a clear and ongoing understanding of patient confidentiality and privacy standards
  • Maintains prompt and regular attendance
  • Performs related work duties as assigned by supervisor
  • Verify timecard at the end of each pay period
  • Completes the required IT/Security trainings prior to the deadline
