About The Position

Under the supervision of the Director of Health and Social Services, the Program Manager is responsible for coordinating, developing, promoting, and implementing a grant; creating policies and procedures; and ensuring overall quality assurance of services. The Program Manager also oversees grant-funded initiatives, represents the organization in the community, and builds relationships with diverse groups and organizations. This role requires maintaining confidentiality and professionalism at all times.

Requirements

  • Bachelor’s degree in human or social service or relevant experience. Master’s degree preferred.
  • One year of experience with and knowledge of Social Determinants of Health (SDoH).
  • One year of supervisory experience, preferably in a non-profit organization.
  • Prior patient navigation or case management experience preferred.
  • Bilingual (English and Spanish) preferred. Good written and verbal communication.

Responsibilities

  • Supervise daily operations, staff, and volunteers to ensure efficiency, professionalism, and effective use of resources.
  • Recruit, train, evaluate, and support staff development, providing coaching and feedback to maintain high performance.
  • Monitor grant activities to ensure compliance with guidelines, achievement of goals, and quality improvement.
  • Create and maintain organizational policies, procedures, schedules, and workflows that support program operations.
  • Maintain accurate records, databases, and reports for program evaluation, quality assurance, and accountability.
  • Build and sustain relationships with community partners, represent and promote program services to other organizations and community groups serving the rural community through representation, community presentations, events, and/or other avenues.
  • Prepare narrative and statistical reports on program progress and outcomes, ensuring alignment with organizational standards.
  • Maintain flexibility for occasional evening, weekend, or emergency coverage to meet program and community needs.
  • Support internal and external meetings, including multi-disciplinary and organizational team gatherings.
  • Maintain established departmental policies and procedures, objectives quality assurance programs, safely, environmental and infection control standards.
  • Enhance professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  • Supervise development, implementation and operations of SYHC’s TQM Program as it relates to patient care.
  • Present reports on operations and/or corporate compliance.
  • Attend meetings as required and participate on committees as directed.
  • Perform other related duties as assigned or requested.
