The Rural Health Transformation Program Manager is responsible for leading the design, implementation, and evaluation of initiatives aimed at improving the health systems in rural communities. This role focuses on care delivery models that extend preventative, primary, and specialty care to rural residents through expanded care teams and telehealth. The Program Manager collaborates across health systems, payers, and community partners to develop rural regionalization plans, implement evidence-based strategies that improve health outcomes and ensure the long-term viability of rural healthcare delivery.

  • Demonstrates knowledge of and supports mission, vision, value statements, standards, policies and procedures, and the code of ethical behavior.
  • Develop and manage strategic initiatives that align with the state’s rural health transformation priorities.
  • Lead the design and execution of rural health transformation initiatives focused on care delivery redesign, financial sustainability, and system integration.
  • Build and maintain collaborative relationships with hospital and clinic leadership, provider networks, public health agencies, and payers.
  • Facilitate cross-sector partnerships to integrate behavioral health, primary care, and social services within rural settings.
  • Serve as a liaison between healthcare systems and community partners to promote data sharing, resource alignment, and collaborative impact.
  • Convene cross-sector workgroups and facilitate community engagement and stakeholder meetings.
  • Use population health analytics and qualitative data to identify priority needs, measure effectiveness, and inform continuous improvement.
  • Use quantitative and qualitative data to assess system performance, identify improvement opportunities, and measure impact.
  • Develop and monitor metrics related to access, quality, cost, and sustainability.
  • Prepare reports, presentations, and grant submissions for internal and external audiences.
  • Manage budgets, timelines, contracts, and staff or consultant activities within assigned projects.
  • Monitor federal and state policies impacting rural health delivery, reimbursement, and transformation programs.
  • Ensure compliance with regulatory and funding requirements.
  • Being present at the office is an essential function of the job.
  • Other duties as assigned.

  • Education and Experience requirements at this level consist of a Master’s degree in business, public administration, public health, healthcare, finance or related field OR a Bachelor’s degree in Public Health, Health Administration, Nursing, Business, or related field required, plus 5 years of progressive experience in healthcare system transformation, rural health, or health policy and demonstrated experience in program design, grant management, and multi-sector collaboration.
