About The Position

The Rural Community Support Provider at The Well is dedicated to empowering individuals with behavioral health needs by offering compassionate support and essential services to those living in rural Nebraska communities. This position primarily provides services in the field by traveling to rural towns and communities, rather than operating out of a centralized hub location in Norfolk or a satellite office in Columbus. While a satellite office space is available in West Point for administrative needs, this role is expected to travel regularly and provide services across multiple rural areas and counties, not limited to Cuming County or the surrounding Region 4 area. This position requires a resourceful and empathetic professional who can effectively assist clients in their recovery journey while upholding our core values.

Requirements

  • Must be 21 or older.
  • Valid Nebraska driver’s license, insurable, and confident driving record.
  • Willing and able to spend the majority of work time traveling to rural Nebraska communities and meeting clients in the field.
  • If in recovery, at least 18 months sober and able to pass random drug screenings.
  • Bachelor’s degree in psychology, sociology, or related human services field preferred.
  • OR 2 years coursework/experience in human services
  • OR 2 years of lived recovery experience with skills in behavioral health.
  • Strong communication and interpersonal skills (empathetic, warm, and supportive).
  • Able to quickly build trust and form positive working relationships with diverse clients.
  • Knowledge of (or willingness to learn) community resources and referral options.
  • Team player who can also work independently.
  • Strong boundaries, problem-solving, time management, and stress management skills.
  • Must maintain confidentiality (42 CFR, HIPAA).

Responsibilities

  • Complete and maintain all required trainings/certifications (e.g., First Aid/CPR, Trauma-Informed Care, Crisis Management, Cultural Competency, Confidentiality, Ethics).
  • Carry a caseload of behavioral health clients.
  • Complete intake, assessments, service plans, and authorizations within set timeframes.
  • Provide consistent support based on program level (Recovery Support, Community Support, Intensive Community Support).
  • Maintain accurate documentation and progress notes.
  • Coordinate with treatment providers, probation/parole/drug court, and other agencies.
  • Complete discharge summaries when clients leave the program.
  • Provide direct support to clients primarily in rural community settings, including:
  • Traveling to meet clients in their home communities.
  • Referrals to services/resources.
  • Assistance with housing, employment, or education.
  • Transportation and support at appointments or court.
  • Medication support and compliance.
  • Crisis intervention and ongoing communication with treatment teams.
  • Teaching life skills (budgeting, coping, relapse prevention, communication, daily living, meal planning, etc.).
  • Encouraging wellness, recovery, exercise, spirituality, and community connection.
