About The Position

Villa St. Joseph was founded in 1997 as a ministry of the Sisters of St. Joseph. After 20 years of service to Beaver and Allegheny counties, the location joined the Concordia family in April 2018 - becoming Concordia at Villa St. Joseph and continuing its faith-based mission of serving others. The RNAC/LNAC is responsible for coordinating and overseeing the resident assessment process in accordance with federal and state regulations. This role ensures accurate and timely completion of MDS assessments, care plans, and related documentation while working collaboratively with the interdisciplinary team to support quality resident care and reimbursement accuracy. Requirements

  • Current RN or LPN/LVN license in good standing
  • Strong attention to detail and organizational skills
  • Ability to work independently and as part of a team
  • Proficient computer and documentation skills

Nice To Haves

  • Previous experience as an RNAC/LNAC or strong MDS experience preferred

Responsibilities

  • Coordinate, complete, and submit MDS assessments in compliance with CMS and state regulations
  • Ensure accuracy of assessments to support appropriate reimbursement (Medicare/Medicaid)
  • Monitor and manage assessment schedules, ARDs, and transmission timelines
  • Collaborate with nursing, therapy, social services, dietary, and other interdisciplinary team members
  • Participate in care plan development and updates
  • Review clinical documentation to ensure completeness and accuracy
  • Assist with audits, surveys, and regulatory compliance as related to MDS
  • Provide education and support to nursing staff regarding documentation and assessment processes
  • Communicate changes in resident condition impacting assessments or reimbursement
