About The Position

Team 8 Covers Northern Eastern Suffolk; Coram, Medford, Middle Island, Miller Place, Mount Sinai, Ridge, Rocky Point, Selden, Shoreham, Sound Beach, Upton, Wading River, Yaphank. Our Home Care RN's: Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. Measure vital signs, Administer IVs, change dressings, clean wounds as needed. Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment. Provide care to the most upmost standard.

Requirements

  • New York State RN license
  • Minimum of one year of recent hands on experience as a Registered Nurse.
  • Basic Life Support Certification
  • Valid New York State Driver's License and reliable transportation
