About The Position

Our rapidly growing in-home senior care agency is seeking a Registered Nurse (RN) to conduct in-home assessments, visit prospective clients, and assist with administrative tasks Compensation & Benefits: Competitive pay and mileage reimbursement. Requirements

  • Current RN license in Virginia
  • A valid driver’s license and a reliable vehicle are required for this position
  • 3+ years of professional nursing experience (Experience in home health, senior care, or a related field is a plus !)
  • Proficiency in the use of computers, internet applications, and software
  • Excellent organizational and time-management skills
  • Ability to multitask in a fast-paced environment
  • Ability to maintain patient confidentiality
  • Can work or visit homes that has pets (both Cat's & Dogs)

Responsibilities

  • Conduct in-home RN assessments on clients in Richmond, VA and surrounding areas
  • Conduct potential prospective visits to assess a client’s eligibility for services.
  • Maintain accurate and organized documentation using our computer systems (must be tech-savvy).
  • Monitor for changes in client's condition and report and document relevant information
  • Assist with the development of an individualized plan of care for each assigned client
  • Supervise and evaluate performance of staff and intervene when appropriate
  • Oversee assigned care levels, services and performance improvement activities
  • Provide recommendations and education to clients and family members on disease process, treatments, medications, and nutrition
  • Communicate and collaborate with healthcare providers and other professionals
  • Ensure cases are managed within defined productivity standards
  • Participate in quality assurance activities, such as client conferences, caregiver and care plan reviews
