Registered Nurse provides nursing care in accordance with the client's Plan of Care which includes performing comprehensive health and psychosocial evaluations, monitoring client’s condition, promoting sound health and preventive practices, and coordinating services, educational/training activities, and direct nursing. How will you make an impact & Requirements Coordinates total p atient care by conducting c o m p r e hensive health and psycho s ocial evaluation, monitoring the c lient ' s condition, promoting sound preventive practices, coordinating services and educational and training activities. Evaluates the effectiveness of nu r sing services to the patient and family. Prepares and presents p a tient's record to t h e Clinical Record Review Committee, as indicated. Consul t s with the attending physician conce r ning alterations of P atient Care Plans, checks with the appropriate supervisor, and m akes changes, as app r opriate. Submits a tally of patient care visits made each day. Participates in case conferences, discusses proble m s concerning t h e patients and how t h ey ma y best be handled with supervisor. Discusses the need for the involve m ent of other m e m b ers of the health team such as the home health a ide, the physical therapist, the speech therapist, the occupational therapist, and the medical social worker, etc., with the app r opriate supervisor. Obta i n s orders for paraprofessional ser v ices and submits a referral to the appropriate personnel. Participates in the patient’s discharge pl a nn i ng p r ocess. Cooperates with other agenci e s prov i ding nurs i ng or related services to provide cont i n uity of care and to implement a co m p rehensive ca r e plan. Participates in s t aff develop m ent m eeting. Continually strives to improve his/her nursing care skills by attending in-service education, through f o r m al education, attendance at workshops, conferences, active participation in p r ofessional and related organiz a tions and indiv i du a l research and read i n g. Participates in the development and periodic revision of the physician ' s Plan of Trea t m ent and processes change orders as needed. Enters clinical n o tes into EMR within required hours of scheduled visit (24 hrs. for non-Oasis visits / 48 hrs. for Oasis visits) and p r ogress notes and other clinical record forms outli n ing the services rendered. Participates in the patient’s discharge pl a nn i ng p r ocess. Maintains an on-going knowledge of current drug therapy. Substitutes for other nurses as requested by Director of Nursing. Complies with Agency dress code policy. Maintains efficient flow of Agency work by reporting to work at assigned or scheduled times; follows procedures for notification of absence or time off requests. Maintains current TB testing (refer to Health Screening Policy).
