About The Position

Lead patient care journeys by implementing and adapting personalized care plans that ensure every clinical need is met with precision and empathy. Champion patient safety through the vigilant monitoring and clear communication of vital status, ensuring the entire medical team is informed and ready to act. Drive multidisciplinary collaboration as the primary coordinator between doctors, specialists, and support staff to deliver seamless, holistic patient care. Empower patients for life beyond the hospital by proactively assessing and organizing discharge resources, ensuring a safe and confident transition back to their community.

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.

Nice To Haves

  • Certification specializing in Wound Care credentialed from the National Alliance of Wound Care and Ostomy (NAWCCB) preferred.
