Job Summary Utilizes the nursing process to provide clinical expertise, consultation, and direct patient care for people with or at risk for fecal and urinary diversions, acute and chronic wound, drains, tubes, fistulas and pressure ulcers. Responsible for developing and implementing a program to promote wound healing by means of educating and training staff, standardizing care, establishment of a comprehensive product formulary, and evaluation of outcomes in these areas. Functions in an educative role (staff in-services, patient teaching and community education), advocacy role, as well as performing research and administrative duties.

Requirements

  • Bachelor’s in Nursing (BSN) required within 5 years of employment if applicable
  • Licensed as a Registered Nurse in the State of Washington
  • Current Healthcare Provider BLS - American Heart Association by start date
  • Board certification as a CWCN, CWON or WOCN through the Wound Ostomy and Continence Society Certification Board within 6 months of hire.
  • Graduate of Accredited Certificate Program in Wound Care Management, or Wound/Ostomy, or Wound/Ostomy/Continence
  • 3 years of clinical practice that includes wound care in Acute Care or Home Health Care setting.
  • Knowledge of Medicare Conditions of Participation, HIM 11, Joint Commission Standards, WACs, WISHA/OSHA.
  • Current Washington State Driver’s license and proof of insurability

Responsibilities

  • Responsibility and accountability for assessment, planning, implementation and evaluation of ostomy and wound patients for patients on service with Evergreen Home Care Services.
  • Functions as a consultant for staff and patients with ostomies, complex wounds, fistulas, ulcers and skin problems related to fecal and urinary incontinence
  • Responsibility and accountability for consultation, direct patient care, program development and education of fecal and urinary diversions or incontinence, and ostomies will depend on level of certification at ostomy and/or continence level.
  • Clinical Expert: Develops and implements assessment and documentation tools, policies and protocols related to wound care in order to standardize practice.
  • Educator: Performs periodic needs assessments related to wound prevention and care. Develops and promotes wound care protocols for EHCS.
  • Acts as a role model and mentor in clinical practice and professional development.
  • Participates in the organization’s committees pertinent to wound care.
  • Bases practice on national standards for wound care, current research evidence.
  • Collects and interprets data regarding prevalence and incidence for the purpose of quality improvement.
  • Performs other duties as assigned.
