About The Position

The Registered Nurse (RN) with Administrative Experience at Silver Living Home Care, LLC is a part-time role focused on providing high-quality patient care in a home care setting. Requirements

  • Current Registered Nurse (RN) license in Pennsylvania.
  • Knowledge of medical records management and documentation practices.
  • Familiarity with acute care environments, including Pediatric Intensive Care Units (PICU).
  • Strong understanding of infectious disease protocols and patient service excellence.
  • Excellent communication skills with the ability to work collaboratively within a team.
  • Ability to assess complex patient needs and develop effective care strategies.
  • Critical thinking skills and the ability to make sound clinical judgments.

Nice To Haves

  • 1 year of nursing experience (preferred).
  • 1 year of case management experience (preferred).

Responsibilities

  • Maintain regulations and licensing for the home care agency.
  • Assess and evaluate patients' needs to develop individualized care plans.
  • Coordinate and manage care for patients within the outpatient setting.
  • Provide education and support to patients and their families regarding treatment options and health management.
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Maintain accurate medical records and documentation of patient interactions.
  • Monitor patient progress and adjust care plans as necessary.
  • Advocate for patients navigating healthcare services, including infectious disease care and acute care needs.
  • Utilize knowledge of physiology to inform case management decisions.
  • Work closely with Level II Trauma Center protocols when applicable.
  • Ensure compliance with healthcare regulations and standards.
