About The Position

The RN Weekend Supportive Care Nurse (day shift) provides one-to-one care at the bedside of terminally ill patients, based on an ongoing nursing assessment of need and in accordance with the established plan of care. Duties include: · Provides compassionate end-of-life care in accordance with the individualized plan of care, with demonstrated ability to assess and respond to patient and family needs in a timely manner · Educate patients and caregivers on end-of-life, services and other resources · Document complex medical information using clinical tools, data and other information to complete assessment of patient needs · Establishes professional and collaborative relationships with patients, families and providers utilizing strong written and verbal communication skills. · Good computer skills, including working knowledge of the Microsoft Office suite · Day shift, part time (24 hours) or full time (36 hours): 8:00 am - 8:00 pm, Saturday and Sunday with weekend shift premium · Care Site - private residences and facilities in Kentucky and Ohio

Requirements

  • Graduate of an accredited School of Nursing and current licensure in Kentucky/Ohio.
  • At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility preferred.
  • Personal qualities of empathy, responsibility and dependability.
  • Must demonstrate effective communication skills with team members, patients, providers, and family members.
  • Must have ability to work independent of direct on-site supervision.
  • Must arrange travel and be able to access client homes, nursing homes, hospitals, and other facilities independently.
  • Must demonstrate proper use of personal protective equipment and observe universal precautions.
  • Must be able to lift at least 50 pounds using proper body mechanics.
  • Must have valid driver's license, current auto insurance and reliable transportation.
