About The Position
Join our compassionate care team! We are seeking a dedicated Registered Nurse to provide high-quality, patient-centered care to individuals in their homes through our Home Health and Hospice services in the Redwood Falls area. This also supports the expansion of Home Health services into the Willmar community, offering an exciting opportunity to help shape care delivery in a growing region. The RN Visiting Care Coordinator is responsible for planning, coordinating, and ensuring continuity of patient care, while maintaining strong relationships with the patient, family, and physician from admission through discharge. Key responsibilities include patient advocacy, clinical competence, and effective resource management. Provide emotional support and education to patients and families. Participate in the development of services in the Willmar area.
Requirements
- Current MN Registered Nurse License.
- Driver’s License required.
- Reliable transportation to make client home visits.
- Strong communication and organizational skills.
- Commitment to compassionate, patient-focused care.
Nice To Haves
- Experience in home health or hospice preferred but not required.
