About The Position

Join our compassionate care team! We are seeking a dedicated Registered Nurse to provide high-quality, patient-centered care to individuals in their homes through our Home Health and Hospice services in the Redwood Falls area. The RN Visiting Care Coordinator is responsible for planning, coordinating, and ensuring continuity of patient care, while maintaining strong relationships with the patient, family, and physician from admission through discharge. Key responsibilities include patient advocacy, clinical competence, and effective resource management. Provide emotional support and education to patients and families. The Community Based Patient Care Coordinator (PCC) uses a collaborative approach through which patient care is organized to support the best practice, desired patient outcomes and efficiency during a patient's episode of care. To be eligible for the hiring bonus you must be an external candidate who is hired into an eligible full-time or part time benefited position. Requirements

  • Current MN Registered Nurse License.
  • Driver’s License required.
  • Reliable transportation to make client home visits.
  • Strong communication and organizational skills.
  • Commitment to compassionate, patient-focused care.
