WOODLAND POND CONFORMANCE STATEMENT In the performance of their respective duties and responsibilities all employees are expected to conform to the following: Performing quality work with or without direct supervision Interacting professionally with other employees, residents, guests, and all others Working effectively and in accordance with the Woodland Pond Communication Fundamentals Completing tasks independently while understanding the necessity for communicating and coordinating work efforts with other employees and organizations Complying with all federal, state and local standards, guidelines and regulations as well as Woodland Pond policy and procedure at all times Position Summary The Assisted Living (“ALR”) RN Unit Manager (Unit Manager of Assisted Living / Enriched Housing) ensures adherence to the Assisted Living regulations applicable to Woodland Pond in NYS, is responsible for training, supervising, evaluating, managing the nurses on the Assisted Living Units of Woodland Pond, acts as primary contact for the residents and loved ones on the Assisted Living Units of Woodland Pond when a clinical/nursing/nursing staff related matter needs to get escalated. Ensures that all staff are productive on each shift and that resources are adequately spread to ensure consistent productivity across shifts. Delegates work appropriately and holds staff accountable. Communicates challenges and opportunities to Director of Nursing early and often. ESSENTIAL DUTIES AND RESPONSIBILITIES (CF indicates Critical Function) Recruitment and Evaluation of Staff Primary responsibility for oversight, management, evaluation and progressive counseling of LPNS and Aides (annual and 90 day evals) and oversight of LPN and Aides day to day on the unit (CF) Prepares and administers all progressive counseling to LPNs and Aides – this includes performance, conduct, time and attendance, as well as any other categories of progressive counseling where appropriate (CF) Consults with Human Resource Director and Director of Nursing for each newly encountered instance of progressive counseling, as needed, to ensure compliance with Woodland Pond employment policies and practices Staffing Notifies DON of changing staff framework needs (increase / decrease coverage for acuity, etc) Unit Manager handles off hours staffing issues only when Scheduler and MOD have not been successful (CF) Medication Administration (CF) Performs a quarterly audit of all med carts to ensure reconciliation of orders versus meds Ensures that all renewals are completed by LPNs timely Follows WP policy related to narcotics and controlled substances Responsible to cover med passes without objection, in the event of an open shift/portion of shift for med pass on both AL and GV, during normal work hours Education and Development Establishes and maintains a proficient understanding of all ALR and Adult Care Facility Regs in accordance with Title 10. (CF) At a minimum: https://regs.health.ny.gov/content/subchapter-d-adult-care-facilities https://regs.health.ny.gov/content/part-1001-assisted-living-residences https://regs.health.ny.gov/content/section-100110-resident-services Oversees and evaluates staff, reinforces lessons learned in training sessions provided and performs random critical thinking audits on all staff Is an active member and contributor to the GV (memory care) programming Care Planning Unit Manager along with Case Manager ensure accuracy of Individual Service Plans and Acuity Tools, communication, and adherence to same (CF) Works with Case Manager to correctly identify residents that should be deemed “Enhanced” level of care on census (40 of 60, permitted) Physician and Family Communication a. Responsible for ensuring that physician communication protocols are designed and operating in a manner consistent with the expectations of the primary physician / groups for AL/GV residents. (CF) b. Ensures that all LPNs are proficient in the use of Tiger Text, and are consistently communicating the appropriate level of information required by the primary physician / groups via Tiger Text. c. Responses to families should be made same day where possible and within 24 hours for all other times except in rare exceptions of circumstanceThis may include ancknowldgement that a further response will be pending, to an inquiry, but at a minimum, and acknowledgment of the receipt of the inquiry before departure daily. d Demonstrate empathy, professionalism, and responsiveness in all interaction with residents and families. e Serve as a point of contact for concerns and ensure timely follow-up and resolution to promote satisfaction and trust. Regulatory Compliance & Quality Assurance a. Ensure all required waivers, regulatory documentation, and internal audits are completed accurately and within required timeframes. b. Conduct routine audits of medication and treatments records to verify accurate transcription, proper documentation, and compliance with physician orders and NYS assisted living regulations. c. Identify, address, and trend areas of non-compliance, implementing corrective actions as needed. Admissions & Pre-Admission Process a Participate in pre-admission introductions to prospective residents and families to ensure appropriate placement and establish a welcoming, supportive transition into the community. b Collaborate with interdisciplinary team members to ensure all admission requirements are met prior to move-in. Unit Oversight, Daily Operations and Leadership Development a Maintain a consistent presence on assigned units (including Garden View and Assisted Living) throughout the day to oversee operations, provide support to staff, and ensure quality resident care. b Conduct daily rounds to monitor resident status, staff performance, and overall unit functioning, addressing concerns in real time. c Foster a positive work environment and reflect the organization’s mission, vision, and values. d Promotes teamwork, accountability, and respect among staff while supporting ongoing education and professional development. e Lead by example in delivering compassionate, resident-centered care. Policy and Procedure Responsible for implementing and enforcing all policies and procedures for all staff, on all shifts, based on having a proficient understanding of the ALR regulations Requesting, as needed – higher level in service support from I/D team if necessary on the implementation and enforcement of these policies Active consideration of existing policies and procedures and proactive requests to appropriate parties for consideration of policy and procedure modifications to better and more safely serve our residents (CF) Order of Accuracy and Compliance Receives daily feedback from LPN’s on missed orders / concerns / delays, etc. (CF) Carefully monitors accuracy, appropriateness and best practice with the inter-disciplinary team and Medical orders (CF) Includes monitoring of all interdisciplinary orders Participate in and contribute to facility QAPI as deemed necessary in ALR Serves on Manager on Duty rotation (CF) Rounds AL and GV from end to end, twice daily (CF) Provides limited support of clinical information systems on a periodic basis Completes all required training and in-services All other duties as assigned GENERAL SKILLS AND ABILITIES Maintains high standards for work areas, attitude and appearance Understands that honesty and ethics are essential in the performance of duties Willing to learn and grow Demonstrates maturity in judgment and behaviors Able to work a flexible schedule Maintains predictable and reliable attendance Degree of Travel Travel required for community activities and hospital or other health facility visits Disruption to Routine Disruption to routine is to be expected frequently.
