About The Position

The Resident Care Coordinator (RCC) leads subacute and long-term care units by ensuring high-quality patient care, supporting staff, and maintaining compliance with regulatory standards. The Unit Manager will be the key point of contact for residents, families, and healthcare teams, ensuring compassionate and effective care.

Requirements

Current New York State Registered Nurse (RN) license (required).
Strong decision-making skills for critical situations.
Ability to supervise and work collaboratively with other healthcare personnel.
Excellent verbal and written communication skills.
Physical ability to lift a minimum of 20 pounds and engage in frequent activity.

Nice To Haves

Experience in long-term care or similar healthcare setting (preferred).

Responsibilities

Oversee nursing care and manage daily operations of the assigned unit.
Maintain and restore the health of resident patients.
Serve as the main liaison between residents, families, and healthcare providers.
Address and resolve family concerns to ensure satisfaction.
Develop, implement, and adjust patient care plans as needed.
Supervise, mentor, and discipline nursing staff as necessary.
Ensure compliance with federal/state regulations and company policies.
Collaborate with the Director of Nursing and healthcare providers to plan and monitor resident care.
Complete admission and discharge assessments.
Audit and oversee updates to care plans.
Provide hands-on care and administer medications as needed.
