About The Position

The Triage Registered Nurse at St. Croix Hospice is responsible for coordinating and managing patient care through phone consultations, ensuring appropriate hospice services are provided. This role emphasizes autonomy, allowing nurses to lead care plans and spend quality time with patients, while also being supported by a collaborative team focused on delivering exceptional end-of-life care.

Requirements

• Required RN licensure for the state of MN and WI.
• One to two years of recent acute care experience in an institutional setting.
• One to two years of recent experience in home care is preferred.
• Bachelor's degree preferred, with one year of hospice care experience preferred.
• Excellent observation, verbal and written communication skills.
• Problem-solving skills and basic math skills.

Nice To Haves

• Management experience not required but preferred.
• Experience in palliative care is a plus.

Responsibilities

• Direct hospice services and care via phone conference.
• Instruct patients and caregivers on appropriate interventions based on nursing knowledge.
• Assess available options for interventions and utilize critical thinking to choose the best intervention.
• Determine the need for nursing visits based on EMR and telephone calls.
• Dispatch visit RN/LPN appropriately based on assessments.
• Facilitate patients' efforts toward self-sufficiency and optimal comfort care.
