We are seeking a Dynamic, self-motivated RN Triage Nurse who thrives in a fast-paced environment and supports our mission. Sign-On Bonus: $1,000.00 for Registered Nurse paid out in 2 equal payments over 12 months Shifts are: 1600 - 0030 or 0000 - 0830 POSITION PURPOSE/SUMMARY: Under the direction of the Team Lead, the Triage Nurse will handle phone calls from RN Case Managers, patients, and triage needs. The Triage nurse is also responsible for the coordination of the on-call staff and patient visit schedules.

Requirements

  • Currently licensed to practice professional nursing in the State of California.
  • At least 3 years of dealing with acute care patients preferred. Six months or more of home care or Hospice experience preferred.
  • Supervisory experience
  • Ability to set priorities, organize and work independently.
  • Ability to learn and assimilate the Allscripts computer software program.
  • Ability to communicate effectively verbally and in writing.
  • Skill in nursing practices with understanding and knowledge of Hospice care and committed to the philosophy and goals of the program.
  • Ability to make independent clinical decisions.
  • Knowledge of medications, indications, dosage ranges, side effects and potential toxicity.

Responsibilities

  • Receives and gives reports daily to RN, LVN, and other pertinent staff as needed.
  • Serves as the Back-Up RN On Call, providing in-person visits when patient care needs exceed the scope of practice of the primary on-call LVN.
  • Assists in receiving new orders from the On Call Physician and coordinating with the facility, pharmacy, family, etc.
  • Provides GIP Death visits as needed.
  • Triages patient calls from families and RN Case Managers.
  • Schedules patient follow-up visits, as needed.
  • Coordinates patient services, including but not limited to CADD pump, Palliative modalities, Transfers, Closes, and Revocations.
  • Coordinates Continuous Care/Inpatient for RN Case Managers when ordered by the physician.
  • Communicates staff updates with the clinical team.
  • Inputs patient data.
  • Conducts an audit requested by DHCS.
  • Responsible for updating DHCS and/or the Team Lead of any changes in the level of patient care, adverse events, and unusual occurrences.
  • Reviews COPs when updates are provided.
  • Provides operational and administrative support to the Team Lead and DHCS.
  • Provides clinical coverage for the Hospice House RN during meal periods, ensuring continuous patient monitoring, timely response to needs, and regulatory compliance.
  • Assists Hospice House RN in emergencies.
  • Performs additional clinical duties and responsibilities as assigned to support patient care and operational needs.
  • Uses expert nursing judgment and functions as patient advocate, in collaboration with IDT and primary physician, when seeking a change in optimal care setting for the patient, incorporating patient safety and family caregiving ability.
  • Maintains up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and kept in accordance with Medicare, Medi-Cal, State Home Health Agency, and Joint Commission standards.
  • Follows universal precautions in delivering patient care.
  • Works with social services to identify emotional, psychological, and spiritual needs of the patient.
  • Makes recommendations to IDT for action/follow-up.
  • Collaborates with physicians to determine appropriate frequency of nursing visits and the need for a change of level of care (continuous home care, respite, attendant care, or inpatient admission, discharge for the Hospice Program)
  • Interventions reflect an advanced level of nursing practice and commitment to the execution of the IDT plan of care.
  • Provides support to families at the time of the patient’s death with coordination of services with the physician, mortuary, DME, and supplier.
  • Assists family in making transitions from caregiving to bereavement, in coordination with IDT.
  • Participates in Quality Assurance and Utilization Review activities as requested by administration.
  • Incorporates results of QA/U.R. studies in nursing practice.
